CRO Organization Referee Request Form

This form is for clubs, teams or other organizations to request referees for individual or multiple matches outside their normal league submission process. Please be sure to read throughly through all information to ensure the most seamless process in requesting and paying for referee on your matches. If you are an organization that is new to work with CRO please contact us through email in addition to requesting referees for your matches.

Organizations looking to submit match requests have three options using the jot form below. This is for submission of referee requests for matches outside of the normal league submission process. If you have already submitted your matches and need to change a match please read through the Match Change Request section and use the specific Match Change Request Form.

Referee Match Request Form

  1. Individual Match Request via Jot Form - This option is for organizations submitting a single match request.

  2. Multiple Match Requests via Jot Form - This option is for organizations submitting between 2-3 matches.

  3. Multiple Match Request with Spreadsheet via Jot Form - This method is for organizations submitting 3 or more matches.

Invoicing Match Fees

Invoices for Referee Match Fees and assignor fees will be sent separate from the confirmation receipt.

Match Change Requests

Organizations needing to change an individual match that has already been sent to us please email with the information below.
