What is a Supplemental Report?
A Supplemental Report provides more detailed information about specific match incidents. This report will provide the league with additional information for any disciplinary actions that must take place or to fix issues with any match day requirements
For what must a Supplemental Report be filled out?
ANY Red Card given during the match, this includes a Second Caution Red Card.
When match day requirements are not met by the home or visiting team
Referee Assault or Abuse
Field Issues
Roster Issues
Field Issues that cannot be fixed but can be played without.
Major Injuries when a player does not return
Head Injuries
Spectator issues

Yes, BUT if you do not address it during the match it becomes extremely difficult to issue discipline to a coach, team or spectator if you did not address it in the match.
